NDEWA Membership offers:

designated to the beneficiary of your choice.
When a member passes away, the
Secretary of NDEWA needs to be contacted. The Secretary will then contact Holly
Parenteau at National NEW. She will complete the statement of
Employer/Association section of the Request for Policy Proceeds claim form and
then forward said form, along with return envelope, to the designated
beneficiary of the NEW Life Insurance policy. The beneficiary will need to
complete the rest of the form and send it and a certified copy, with the raised
seal, of the claimant's death certificate to the Horace Mann Companies. The
claim payment will be generated directly to the beneficiary.
